Feedback on a reported quality issue will be provided through the Customer Service Section at the MSD zone serving your facility.

The MSD Quality Complaint form can be obtained from the Customer Service Unit at the respective MSD zone where your facility is located. The completed stamped and signed form, should also be submitted to the same zone.
Immediately report the issue by completing the MSD Quality Complaint form with full and accurate details, clearly describing the problem encountered.
